Abstract

Immunization is the most effective and affordable primary preventive measure to prevent infectious diseases. The coverage of basic immunization at the Simang Agung Health Center in 2024 was recorded at 28.7%. One of the factors influencing the low immunization coverage is the level of knowledge through the Mother and Child Health Book (Buku KIA). The MCH Book is a communication and education tool that serves as a guide for mothers during pregnancy, childbirth, postpartum, and child development. The purpose of this study was to determine the relationship between the utilization of the MCH Book and the level of compliance of mothers in administering basic immunization to infants in the Simang Agung Community Health Center working area, Central Lampung Regency, in 2025. This is a quantitative analytical study with a cross-sectional analytical design. The population in this study was all mothers who had babies in the Simpang Agung Community Health Center Working Area in Central Lampung Regency, with a sample of 85 mothers using total sampling techniques. The study was conducted in July-August 2025. Data collection was performed using observation forms. Univariate and bivariate analyses (chi-square test) were conducted. The utilization of the Maternal and Child Health (MCH) Handbook in the Simpang Agung Health Center area in 2025 remains low, with 54.1% of mothers not using it optimally. Compliance with basic infant immunization is also low (56.5% non-compliant). There is a significant relationship between MCH Handbook utilization and immunization compliance (p < 0.001; OR = 6.364). Mothers need to be more proactive in seeking information and recording immunization schedules through the MCH Handbook while regularly attending Posyandu services. The health center is advised to enhance education through direct counseling, strengthen interpersonal communication, and implement digital reminder systems to sustainably increase immunization coverage.
